Complex Care Plan Development
Complex Care Plans are developed by a Registered Nurse in consultation with the participant, their family or representative(s) and any relevant medical & allied health professionals. Content is tailored to meet the specific and individual needs of the participant and is designed to provide information to guide disability support workers to promote best healthcare practice(s). The development process incorporates evidence-based nursing assessment, evaluation, strategies, and recommendations.
Complex Care Plans require reviewing every 12 months to maintain currency.
